James scanned and posted this at my request. Ron Dethlefson's first BA book contains scans of many of these booklets, but not the Polk Millers. However, the book does contain a scan of a period ad with detailed descriptions of all 4 records. The text of the ad for Watermelon Party matches what's in the booklet above. I would assume that's probably true for the other 3 as well.

The book I mentioned has reproductions of hundreds of those little booklets. Here's a link to one place to purchase. Thanks James, I downloaded the images and set them up in Microsoft Word, adjusted the sizes and printed double sided. and cut it out with scissors. Now I have a nice insert for the reproduction record that I bought from Rob At Edisonia.
