I just looked inside my copy of the book (the yellow and brown hardback published in 1978). Inserted in the book is the same sheet as in your picture. Except that mine has Al Sefl's name and address at the bottom right, instead of George Frow.

I also have the blue softcover predecessor from 1970, by George L. Frow alone.
