That's a "Capital Cuff" music box, made in the late 1800's by F. Otto & Sons in New Jersey. It is a unique hybrid cylinder/disk machine, intended to evade Regina's patents. Otto also made Olympia, Criterion and other regular disk music boxes that got him into a patent infringement war with Regina.
The most prized Cuff machine is a double-comb model F, which goes for $25,000 and upwards. Only a few dozen of them were ever made. The one shown is the smaller model A or B and is more readily available.
