The word "fiend" was another word for addict 100-120 years ago.

Yes it was. This is popular comic strip of the same period...

I love McCay's work: I did an homage to it some years ago.

BTW, that tune you mention has an interesting history, having been released under several different titles, including "The Brooklyn Cakewalk".
