He searched and found the ID plate for the Quadraphone.
As pictured in another post, it sat between the four turn tables with the tonearm rising from the center of it.

As a result of selling the data plate, the buyer informed me that a upright Quadraphone exists intact in Holland.
He sent me these photos.
