Here's a SM that was made for Edison Bell.

Upon the sale, lease or other disposition of this phonograph, due notice of above conditions must be given to the purchaser.'

I'll assume this plate covers the Edison signiture.

Check your item 3 again. Our machines are relatively close (yours being older by the smaller S/N) and I originally thought the date was 1908, but when I looked under bright light, it was 1906. I went back and took the below picture.

George, could an older machine have a plate with a later date than a younger machine of the same model?
