Yes, it's perfectly legit! It looks like it's from a "Lakeside" machine sold by Montgomery Ward, circa 1911. It was coupled to a double-diaphragm reproducer to play either 4 or 2-minute cylinders. Described in The Taking Machine Illustrated Compendium Book, page 187!
