The Talking Machine Forum — For All Antique Phonographs & Recordings

A collector has sent me some grainy pics of this disc. Can anyone identify it? I can't seem to find any info on it. Thank you.

I suspect it's the same recording issued on the Nation's Forum green/red disc, with James Gerard's address on the other side. But this format, with a gold "Tiffany" label, is unknown to me.
