I don't know the model name, but it looks pretty similar to this one?

Some that are very close, but still not completely accurate:

I have one of these in my garage that I had electrically restored by Philip Knighton about 12 ish years ago. Unfortuanately I also had the cabinet restored by someone else who had their own ideas about what the finished result should look like :( I wasn't happy with the result and consequently its lived its life ever since in various garages over the years. I don't think it still works but I don't think it would be difficult to get it working properly again. I recall Philip spent many hours re-building the amplifier. I've been wondering what to do with it for a while now - so if you need the electrics from one to put in that cabinet then you would be welcome to it for free.

PS the amp in this would of originally contained a PX4 valve / tube but when I got the machine that was fried and due to the cost of an exact replacement as it was being restored I opted to have a substitute put in instead. Does the same job but just doesn't attract the ridiculous price of a PX4.
