Does anyone have a scan or clear photo of the cabinet-bottom license label " Form 1361-A " for a mid-Teen's Grafonola upright?
I have recently acquired a new Grafonola, and the only shred of the original license label is a tiny bit from the top left, enough to read: " Form 1361-A "

I am not sure if this is what you are looking for. By coincidence, I just used my iPhone to try to take a picture of the label on the underside of the Columbia 200 Grafonola I recently placed in the Yankee Trader section of the TMF. I wish it was clearer and more detailed. Good luck!

Not sure if this is the same form, but this is the general label in question !
My machine is a mid-Teens upright, probably a Nonpareil or "200"... cabinet is the same design as your Am Walnut, but in red/brown mahogany. The neat part is that it has the original Columbia "Universal" electric motor.
I have been pining and drooling over your Walnut 200 !
