Interesting Cylinder Record Case from 1970??

I was looking through some cylinder records and found an interesting cylinder case for Sargent Cylinder Recordings from 1970, I think. The address has a zip code and the phone number has an area code. Any idea what this is? It contained remnants of a recorded brown wax cylinder.

I never had one, but I recall them being advertised in hobby publications. They were dubs, I think one of them was the "In am the Phonograph" cylinder.
