Just an update. After waiting a considerable amount of time I found out that the copy bought from Amazon UK had been lost in shipment. Amazon refunded the purchase price.
While waiting for this to play out, Mr. Baumbach (responding to my earlier request to Mullholand Press) let me know that he had one copy left, which I have just bought. He also indicated that he had no plans to reprint the book or to issue it as an eBook at this time. He is preparing to issue an eBook version of his book on Edison cylinder phonographs.
