advisory tag on bottom of Columbia BNWM disk machine

I have a Columbia BNWM that has an advisory tag on the bottom. This tag is only partial, so I would like to have a complete one. Does someone out there have such a tag that is complete, where you could email to me as a jpg or pdf facsimile? If so, use mccsc2 at gmail dot com

I have a BNW; no -M, but I think it's same thing.

This is a photo (lo-res for web) at best of my skills and my camera (an iPhone).
I've e-mailed you the photo at native resolution.
I think it couldn't be difficult rebuilt the "notice" with a computer. Its dimensions are 20 cm (lenght) x 15,5 cm (width).
