Wanted "Victor Remove Plate"

I would appreciate if someone could sell me the instruction plate "Remove Plate" for my Victor M front mount machine. It measures 3 ½ in X 2 in.
From across the room, this cardboard photocopy looks pretty good, but it's just not right.
