From a Silvertone. The names indicate which of the two holes on either side of the large hole to store the jeweled styli -- sapphire ball and conical diamond.

Re: Metal plate / tag?
Most non-Victor/Columbia/Edison disc phonos made between 1916-1919 had some provision for playing both lateral and vertical records either via a swiveling reproducer or interchangeable reproducers. A conical diamond mounted to a shank was used for Edison records; a ball sapphire for 'hill-and-dale' records such as Pathé, Rex, etc. When not in use, the jeweled styli were stored jewel up in those two holes.

The central hole was used for the fitting that changed the soundbox from lateral to vertical orientation.
